Insurance and Payment Options
Patients who are not covered by health care insurance should remember that we expect payment in full (or appropriate payment arrangements) for all services at the time of each visit. You may pay by cash, personal check, VISA, MasterCard, Discover, American Express, Optima, or MedCash.
We accept reimbursement from Medicaid (Kentucky Medical Assistance Program, including the Medicaid Managed Care Partnerships) as payment in full for our services which are covered by Medicaid.
We accept assignment for TRICARE (formerly CHAMPUS) and CHAMPVA. We will file your claim and then send you a statement for your cost-share, any unmet deductible, and any non-covered services after claim processing is complete.
We are participating providers (that is, we accept assignment for all services) with Medicare Part B. We will file your Medicare claim, along with any Medicare supplement, and send statements for portions that are your responsibility. We will collect 20% of the Medicare allowed amounts for services rendered if there is no supplemental insurance. We will also collect any unmet deductibles on the date of service if applicable.
If you have a PPO or HMO office visit co-payment, we expect the co-payment at the time of each visit. Otherwise, we will file your claim and then send you a statement for your co-insurance, any unmet deductible, and any non-covered services after the claims have been processed.
For all other insurance plans with which we do not have a formal participation agreement, , we will file your insurance claim and then send you a statement for your balance after the claims have been processed.
***PLEASE NOTE: It is always your responsibility to ensure that you have a current and valid referral form (or authorization) from your primary care physician if your insurance plan requires a referral form (or authorization) for medical and/or surgical specialty care.
LASIK PATIENTS: Commonwealth Eye Surgery will honor MOST insurance discounts offered towards LASIK/PRK surgery if written documentation of coverage and any such discount is provided. Such discounts are granted regardless of whether we are listed as a “participant” or “preferred provider” of a particular plan or not. Many insurance plans are starting to offer discounts towards refractive surgery, so we encourage you to check with your vision insurance carrier in order to maximize the benefits extended to you by your plan.
