Insurance and Payment

Patients who are not covered by health care insurance should remember that we expect payment in full (or appropriate payment arrangements) for all services at the time of each visit. You may pay by cash, personal check, VISA, MasterCard, Discover, or utilize Care Credit for special financing.

We are participating providers (that is, we accept assignment for all services) with Medicare Part B. We will file your Medicare claim, along with any Medicare supplement, and send statements for portions that are your responsibility. We will collect 20% of the Medicare allowed amounts for services rendered if there is no supplemental insurance. We will also collect any unmet deductibles on the date of service if applicable.

We participate with Kentucky Medicaid as well as two of the MCO plans, Humana Medicaid and Anthem Medicaid. We are not part of the UK or ARH HMO network, but we will honor their insurance discounts should you choose to have your surgery at Commonwealth Eye Surgery.

At the time of your visit, we will collect your estimated responsibility based on the insurance benefit information provided by your insurance provider. If you have two or more insurances, we generally will not collect at the time of your visit, but you may still receive a bill after services are rendered based on your insurance processing. We request that you review our estimates carefully and notify us if anything appears inaccurate.
